Duties Description • Supervise Home Liaison staff in the development of Individual Habilitation Plans, implementation, Review staff actions Plans and Res. Hab. Data sheets, establishment of goals, and monitoring Individuals’ progress toward the achievement of those goals. Ensure that the programs meet all applicable Federal, State and local regulations.
• Ensures that Home Liaison staff upload all Life Plans, Staff Action Plans and all other paperwork deemed necessary to the HER.
• Work closely with the residential clinical team to ensure coordination of services for Family Care Program.
• Responds to crisis situations and emergencies involving Individual’s and provides leadership to assist staff and Individuals during such times.
• Maintain certification according to regulatory requirements for Family Care and HCBS Res Hab.
• Liaison with Voluntary Operated Family Care Agencies in the certification and provision of services to Individuals living in voluntary operated Family Care homes.
• Ensure Family Care Providers receive training and maintain certification.
• Meets with and instructs staff regarding work projects, program objectives, delivery of services techniques and the implementation of OPWDD policy.
• Establishes standards of performance, observes staff visits and activity, determines the efficiency of the staff and programs being delivered.
• Reviews Individuals records to determine if proper and adequate information is continuously updated.
• Participates in meetings with various DDSO and Agency clinical/support staff in order to provide and exchange program and/or administrative information.
• Make Family Care Home Evaluations and survey visits, Observe and review fire drills as needed.
• Ensure that all monthly reviews, acumatica audits are completed. Ensure Individuals annual/semiannual Life Plan attendance by Home Liaisons and Family Care Providers.
• Provide specialized training, when required, and guidance to staff on a day-to-day basis.
• Ensure community inclusion activities for each Individual and the delivery and documentation of community inclusion services in accord with each Individual’s “valued outcomes”.
• Ensuring the appropriate and timely reporting of family care incidents, including the completion of required forms, and required follow-up.
• Ensure the completion agency assigned investigations and investigative reports (and follow-up when required) for each reportable incident.
• Communicate and review all pertinent information with Supervisor in a timely manner.
• Additional duties as deemed necessary by the Family Care Coordinator/ TTL/DDP4 or Agency Director.

Minimum Qualifications Promotion: one year of service as a Social Work Assistant 1.

Open Competitive:
A bachelor's degree in a human services field and two years of social work experience.

Substitution:
Master's degree in a human services field can substitute for one year of specialized experience.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• Please note that starting salary for this title reflects the downstate adjustment. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 17 ($63,361) and Downstate Adjustment ($4,000). Downstate Adjustment is added to the job rate ($80655).

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.

• Preference will be given to OPWDD employees impacted by closures. If you are being impacted by closure, please indicate this on your resume.

• All OPWDD employees must be eligible and maintain eligibility for full and unconditional participation in the Medicare and Medicaid programs. Continued employment will depend on maintaining eligibility.
